Death History Search Form



To order a copy of a record 1892-1922, send an e-mail request. We will confirm and indicate the actual fee by e-mail. The basic fee for searching and copying records is $10.00 for a plain copy and $15.00 for a certified copy.
Records from 1923 and later, please contact Maine Department of Human Services, Office of Data Research and Vital Statistics at 207-287-3181 or U.S. mail at 11 State House Station, Augusta, ME 04333. As of July 12, 2010, Maine is a closed record state (pursuant to MRSA Title 22 Section 2706) and identification is now required in order to obtain a vital record. If the record you are requesting is not an open record (Birth - 75 years or older; Death - 25 years or older; Marriage - 50 years or older) you must show proof of direct and legitimate interest.
All checks (sent to either the Maine State Archives or to the Department of Human Services) should be made payable to: Treasurer, State of Maine. Please enclose a self-addressed, stamped business envelope with any orders.

This database, created as a convenient index to original records, may not reflect the official information on those records. To correct an error, contact the Maine State Archives (see below). We will check the official record, then make changes that match the information on the record.
